Does it hurt?
Not any more. When it did hurt it was pretty minimal (i.e. an Advil took care of it)
How long after the piercing does it hurt?
It hurt the most when I went from a 10 gauge to an 8 gauge, but that's only because I didn't wait long enough before switching. Even then it only hurt for a few days.
Is it as bad as getting your nose pierced?
I'd say worse, because it's a bigger gauge needle. I had my second piercing done with a 12 gauge and that definitely hurt worse than the 18 gauge nose piercing.
Is it as bad as getting your nose piercing stretched?
I've only ever stretched my nose from an 18 to a 16, which didn't hurt that bad, but the bigger you go up in gauges, the more it's going to hurt.
